General Statement of Job

The Transition Coordinator will be responsible for assisting incarcerated individuals in their journey towards reentry and recovery by implementing an individualized case plan for addiction recovery via community partners, housing, employment, substance use and mental health support, family/friend preparation support, and transportation assistance, all while providing advocacy, guidance, motivation, and knowledge.


Minimum Qualifications

  • High School Diploma, GED or equivalent.
  • Applicants must have two (2) years of clerical or corrections experience.
  • Applicants must possess and maintain a valid driver’s license.
  • Applicant must be able to prioritize tasks based on level of importance.

Essential Job Functions

  • Regular and prompt job attendance.
  • Operates computerized management information systems to retrieve and enter information.
  • Proficient in NCIC/FCIC.
  • Capability to read and understand Criminal Histories.
  • Analyze, enter, and track recidivism.
  • Interpersonal and motivational interviewing skills.
  • Plan, organize, and coordinate placement into post-release community treatment programs, housing, etc.
  • Assist with discharge planning to include post-release transportation needs, housing needs, educational needs, vocational needs, and mental health needs of the inmates.
  • Assist inmates with obtaining ID Services, such as, DL/ID cards, social security card replacement and certified birth certificates, etc.
  • Maintain radio communications with detention deputies in housing modules, communicate with inmates via intercom, answers telephone calls, and relates information to authorized persons.
  • Completes all assignments to specifications within a specified timeframe and budget as directed and in accordance with agency policies and regulations.
  • Performs other related duties as assigned.

Required Knowledge, Skills, and Abilities

  • Knowledge of various computer systems to include CJIS, NCIC/FCIC, and JMS.
  • Knowledge of community/state-based services available to incarcerated individuals.
  • Knowledge of HIPPA laws.
  • Skill in the use of computers, two-way radio communications, intercom system, and a variety of detention devices used to perform daily functions of position.
  • Effectively prepare reports and logs.
  • Effective verbal and written communication skills.
  • Maintain composure during crisis or emergency situations.
  • Maintain concentrated visual attention to detail for an extended period.
  • Prioritize and exercise good judgment under stressful circumstances.

Physical Abilities

  • Sit up to 6 hours per day
  • Stand up 1 hour per day
  • Walk up 2 hours per day
  • Frequently lift up to 10 lbs.
  • Occasionally lift up to 20 lbs.
  • Frequently bend, reach, or twist
