General Statement of Job
This position is under the direction of the Outreach Program Manager. This position provides assessment, discharge planning, and individual case coordination to participants re-entering the community from the Walton County Jail.
Minimum Qualifications
- Bachelor’s degree in Criminal Justice, Counseling, Psychology, Social Work, or related field.
- Two (2) years work experience in the mental health field or equivalent combination of education and experience.
- Must possess a valid driver’s license.

- Regular and prompt job attendance.
- Follow all standards and participant care protocols and the Walton County Sheriff’s Office policies, regulations, and guidelines.
- Collect data about participant through interview, case history, and or/observation techniques.
- Assist in the development and implementation of a re-entry care plan.
- Provide support and case management to participant through connection to services in the community to ensure access to housing, healthcare, employment or educational opportunities and other general living and recovery.
- Provide timely, documentation for all service outreach attempts in accordance with agency/regulatory standards requirements.
- Facilitate communication between various community providers on behalf of individuals.
- Develop and maintain a network of trusted contacts of organizations and agencies in which to refer and coordinate care.
- Meet regularly with Program Manager as a means of enhancing professional growth, reviewing, and evaluating the provision of case management services, and attend to required administrative issues.
- Maintain ethical standards and immediately communicate issues of safety and/or individual risk issues to program manager.
- Participate in assigned trainings to develop and maintain professional capabilities and proficiency.
- Attend mandatory and training as required.
- Perform all other related duties as assigned.
Required Knowledge, Skills, and Abilities
- Knowledge of community resources.
- Knowledge of and experience working with local behavioral health, social services and criminal justice systems.
- Skill in establishing rapport.
- Skill in effective writing.
- Ability to work effectively with a diverse community and value cultural differences.
- Ability to work independently and in a team.
- Must be highly organized, detail oriented, and reliable.
- Ability to work with a high risk/high needs population.
- Crisis management skills preferred.
Standards to Perform Job Functions
Physical Requirements:
- Sit up to 6 hours per day.
- Stand up 4 hours per day.
- Walk up 4 hours per day.
- Frequently lift to 10 lbs.
- Occasionally lift to 20 lbs.
- Frequently bend, reach, or twist.
